Placement: VHA Home HealthCare appoints Carolelina San Jose as new Director, Professional Practice, Clinical Education & IPAC
Mirams Becker is thrilled to share that Carolelina San Jose has been selected as VHA Home HealthCare’s new Director, Professional Practice, Clinical Education & IPAC, beginning June 29th.
VHA Home HealthCare (VHA) is one of the largest not-for-profit providers of publicly-funded home care in Ontario, with a team of over 3,000 homecare specialists providing personal support, nursing, rehab and community support services in Toronto and the GTA as well as in London, Kingston, and Ottawa.
As the new Director, Carolelina will provide strategic direction for professional practice and education of the clinical staff/service providers with a focus on practice, education, safety and quality by fostering a culture of continuous learning. She will be responsible for building and strengthening the professional practice team, empowering them to design innovative, system-level solutions that respond to the evolving health care environment.
Carolelina is a Registered Nurse with more than 15 years of experience in surgical, intensive care, community, and integrated care settings. She has held several leadership roles, including Clinical Educator and Manager of Quality, Safety, and Risk at Michael Garron Hospital. Most recently, she served as Director of Integration at Health Access Taylor-Massey and Lead for Integrated Neighbourhood Care and Systems Partnerships with East Toronto Health Partners (Ontario Health Team). She is passionate about advancing health equity and improving access, quality, and outcomes through person-centred care.
Carolelina holds a Bachelor of Science in Biology and Psychology from the University of Toronto, a Bachelor of Science in Nursing from Toronto Metropolitan University, a Master of Nursing from York University, and a Master of Science in Quality Improvement and Patient Safety from the University of Toronto. She has also completed a Healthcare Leadership Certificate through the Rotman School of Management.
