Who We Are
Mackenzie Health is leading the development of the Mackenzie Health Academic Family Health Team (MH AFHT), a new primary care clinic, based on an interprofessional team model, serving Western York Region. The MH AFHT will improve access to comprehensive, team-based primary care, advance patient attachment, and support education, research and innovation in partnership with the Western York Region Ontario Health Team (WYR OHT) and academic partners.

Approved by the Government of Ontario in July 2026 and expected to begin welcoming patients in January 2027, the MH AFHT represents an important investment in the future of primary care for the rapidly growing communities of Vaughan, Richmond Hill and King. Currently, fewer than seven per cent of residents in Western York Region have access to team-based primary care, creating a significant opportunity to expand access and build stronger connections across the continuum of care.

The MH AFHT will bring together family physicians and allied health professionals, including social workers, dietitians, physiotherapists and others, to provide coordinated care that responds to patients' health and social needs. An integrated electronic medical record and shared digital tools will help connect information across care settings, strengthening collaboration, continuity of care and the experience of patients and caregivers.

With a strong academic mandate, the MH AFHT will also serve as a teaching clinic affiliated with the future York University School of Medicine, helping to educate the next generation of family physicians while advancing research, innovation and new approaches to integrated primary care.

Together with Mackenzie Health, the WYR OHT and its community and academic partners, the MH AFHT has an opportunity to build a new model of accessible, connected and innovative primary care for one of Ontario's fastest-growing regions.

Position Summary

As the inaugural Executive Director, you will have a unique opportunity to build a new academic primary care organization from the ground up and establish the foundation for its long-term success.

Reporting to the MH AFHT Board of Directors, you will bring innovative thinking, a broad health system perspective and an entrepreneurial approach to shaping the organization, its culture and its model of care. You will translate an ambitious vision into action, building the team, infrastructure, partnerships and operating environment required to launch and grow the MH AFHT.

Central to your success will be your ability to build trusted relationships across a complex and evolving health system. You will forge strong partnerships with physicians, Mackenzie Health, the Western York Region Ontario Health Team, community organizations and system partners, while developing an important academic relationship with York University and its future School of Medicine.

This is a rare opportunity for a collaborative and forward-thinking leader to create something new: establishing an organization designed to expand access to team-based primary care, strengthen integration across the health system, and advance education, research and innovation for the communities of Western York Region.

Key Responsibilities
• Provide overall leadership for the start‑up and implementation of the MH AFHT, including clinic launch and phased expansion
• Operationalize the AFHT’s strategic priorities in alignment with Ontario Health, the WYR OHT, and Mackenzie Health
• Support and work closely with the Board of Directors, including reporting, performance oversight, and risk management
• Oversee clinical operations and ensure delivery of high‑quality, team‑based, patient‑centred care
• Lead workforce planning, recruitment, onboarding, and retention of physicians, interprofessional staff, and administrative teams
• Build and maintain strong partnerships with Mackenzie Health, OHT partners, academic institutions, and community organizations
• Oversee financial management, accountability, and reporting in accordance with funding agreements and governance requirements

Qualifications
• Senior leadership experience in primary care, community health, or integrated health systems
• Strong understanding of Ontario’s primary care landscape, including Family Health Teams, IPCTs, and Ontario Health Teams
• Demonstrated experience leading complex, multi‑stakeholder initiatives or start‑up environments
• Proven ability to work effectively with physicians, boards, and interprofessional teams
• Graduate degree in health administration, public health, business administration, or a related field preferred
