Regional Director, Integrated Client & Community Care (Toronto Central)
Reporting to the Senior Director, as a key member of the leadership team for Integrated Client & Community Care (IC&CC), the Regional Director, IC&CC is accountable to provide leadership and oversight to a diverse team to ensure the delivery of high-quality care in one or more defined geographic areas. This portfolio may include new integrated care programs, clinics, community programs, nursing, personal support, and therapy services. The Regional Director champions integrated client and family centred care and expects high-quality client outcomes and experience, staff and provider experience, clinical practice oversight. In addition, the Regional Director will be primarily responsible for meeting key performance indicators, continuous quality improvement, team development, setting and achieving regional and department goals, and promoting a culture of respect for diversity and promoting “one team” approach.
The Regional Director will be a key champion to advance initiatives related to new models of care, operational improvements, and improving capacity at point-of-care, that aligns with VHA’s strategic plan, portfolio priorities and Ontario’s health system transformation agenda. The role ensures operational processes are reliable and consistent and supports the monitoring of key operations relating to all aspects of service delivery, leadership development, and human resources utilization. The role also applies a project management lens and engages supports and expertise from across the organization for successful design, evaluation, and implementation of initiatives.
In addition, the Regional Director is accountable for building and maintaining strong and collaborative relationships both within VHA and with key regional external stakeholders. The Regional Director will be responsible for building relationships with potential new partners and funders within their region. Key stakeholders for the role include, Home and Community Care Support Services organizations, Ontario Health, Ontario Health Teams, hospitals, primary care networks and other community providers.
Key skills, experience and behaviours required for this position:
- Graduate of a recognized university program in a health-related field. Masters required.
- Current registration with a designated regulatory College in Ontario required, nursing preferred.
- Minimum of 5 years of progressive leadership experience in healthcare working with dynamic and diverse teams, preferably in Home and Community care.
- Proven ability to work across sectors including Home and Community Care Support Services (HCCSS), Ontario Health Teams, hospital, and community support service organizations.
- Knowledge of the social determinants of health and impact on vulnerable populations.
- Demonstrated excellence in ability to think strategically, act resourcefully and demonstrate creative problem-solving.
- Ability to energize and effectively co-create a shared vision with management teams.
- Superior knowledge of clinical practice and leadership from an interprofessional perspective and current health system priorities and transformative planning.
- Ability to make data driven decisions and use of analytics and business intelligence.
- Effective team building and staff development capabilities.
- Effective change agent and thought leader.
- Ability to develop and work within a budget.
- Knowledge of Continuous Quality Improvement processes.
- Skills and experience in customer service techniques such as de-escalation and dispute resolution.
- Demonstrated ability to lead program development and implementation.
- Superior writing skills.
- Excellent interpersonal, verbal communication skills.
- Strong planning; prioritizing, problem solving and project management skills.
- Good computer skills and technology knowledge including word processing and spreadsheet applications.
- Demonstrated interest in professional self-development and maintenance of membership in the appropriate professional association(s) and respective College.
